Perforation시에도 chest PA가 정상일 수 있다

Spontaneous Epigastric pain을 주소로 내원한 77/M pt.

Abdomen 에 diffuse 하게 DT, RT 소견

Other peritonitis r/o perforation으로 chest PA 촬영하였으나 Subphrenic area에 free gas는 없음

--> Perforation시 반드시 Chest PA 상에서 Free gas가 보이는건 아니라고....

80%정도만 보인다고 한다, 내 환자는 나머지 20%인건가

previous Gastric ulcer Hx 있어서 Ulcer perforation 의심하여 Laparotomy 후 subtotal gastrectomy with Billroth II

시행 후 현재는 환자 coma state로 Surgical ICU에 있음
